ADULT DAY CARE SERVICES

        Adult Day Care Services provides a program of activities within a protective setting, to four or more functionally disabled adults who are not able to function in other less structured environments, such as a senior center. An adult day care center provides group care and supervision during the day in a safe, comfortable environment in which selected services are offered by professional and paraprofessional staff. The dominant service objective is to maintain a person in the community and increase access to recreational and social stimuli. The Center also provides dispensable respite to caregivers.

        The following specialized services are representative of the activities that are provided through an adult day care center:

1. Assisting in performing the basic tasks of everyday living, including personal hygiene and use of leisure time;

2. Providing a planned program of social, recreational and developmental activities geared toward meeting the needs of the individual clients within the day care facility, and toward improving or maintaining the functioning level of clients;

3. Referred to and advocating for specialized health, therapeutic, rehabilitative or social services;

4. Providing for nutritious meals and snacks;

5. Working with the client, family, caretaker or other appropriate agency to arrange for transportation.

SPECIAL NOTE

        The provision of adult day care services requires licensing by the Pennsylvania Department of Aging. Only those contractors holding valid licenses will be considered.

        All services must be provided in strict conformance with Pennsylvania Department of Aging regulations (see attached).

        The Area Agency on Aging is especially interested in the quality of the therapeutic activities provided at the Day Care Center, and will carefully review those sections of the proposal which address the types of activities provided, the qualifications and training of staff who plan for and provide these activities, the space and materials available for activities, and how these activities will be monitored to assure integration between the activity plan and the total plan of care for each participant.

        Referrals for adult day care service funded by the AAA will be made directly to the AAA. The AAA will retain responsibility for the initial assessment of need through its Assessment Unit.

        As a result of this assessment, a detailed individual care plan will be developed for each recipient. Each plan will be developed with the informed consent, approval and assistance of the day care recipient or family, who will have the opportunity to identify needed service activities, days and hours of service, etc.

        The AAA will identify service parameters (maximum days/week). The approved care plan will then be forwarded to the contractor for implementation. The contractor shall then schedule service with the service recipient in conformance with the approved care plan.

        In cases where resources do not permit service delivery to all persons in need, the normal AAA priority system will be utilized. (Note: all persons will receive a priority rating at the time of intake. The original rating may be revised as a result of subsequent needs assessments). Contractor is strongly encouraged to have a system set up whereby recipients can pay privately for days beyond those approved by the AAA, if recipients so desire.

        The program will be coordinated with all other relevant long-term services and service providers within the community. This includes recipient referral procedures and all other aspects of the service delivery process.

ADDITIONAL INFORMATION:

1. Service area to be covered is Luzerne and Wyoming Counties.

2. The unit of service for adult day care is a "client day". Client days are defined and must be reported and billed only in increments of half or full days. Four hours or less of a given day shall be reported and billed as one half day. More than four hours on a given day shall be reported and billed as one full day.

3. Type of contract will be "fixed rate per unit".
